We would like to welcome you to our practice. We look forward to working with you to improve your oral health. In order to serve you better and make your initial appointment more efficient, we have provided you with our new patient registration forms (click the New Patient Link to the Left). Please fill out all the forms and bring them with you to your first appointment.

Cancellation Policy

We make every effort to make your appointment at the most convenient time for your schedule. Our doctor, assistant, and/or hygienist are waiting to serve you at your appointment time. We recognize that there are times when urgent situations require last minute appointment cancellations. With sufficient notice, we can fill that time and reschedule your appointment. We ask that you provide us with at least 24 hours notice if you need to cancel your appointment. If you need to cancel over the weekend or after business hours please call our office. Follow the prompts if you have a scheduling concern. We will get your message, and will cancel your scheduled appointment. We strive to provide you with the best dental care possible, and appreciate your efforts in giving your dental appointments the priority they need.

Our Clinic is committed to protecting your privacy. We have never shared nonpublic personal information, except as permitted by law, and we will continue to protect your privacy. We do not share information with outside companies. The following policy serves as a standard for all our employees for collection, use, retention, and security of nonpublic personal information.

• Informational forms filled out in connection with your request for services from our office. This includes your health, insurance, and account forms
• Information we receive from insurance companies or credit companies

  “Nonpublic personal information” is information about you that we obtain in connection with providing dental treatment and service to you. For example: information regarding your health history, dental health, account balance, and payment history.

We do not disclose any personal information about our patients or former patients to anyone, except as permitted by law.
